A form integration gives you a private webhook address. Any tool that can send a form submission to an address, such as Zapier, Make, Elementor or Contact Form 7, can send its leads there, and each one becomes a contact in Relivo.

## Create the integration

1. **Open Form integrations**

    Click **Capture** in the sidebar, then **Form integrations**.

2. **Start a new integration**

    Click **New integration**.

3. **Name it and say where leads come from**

    Enter a **Name**, such as "Facebook lead ads". Under **Sent from**, choose **Webhook**, **Zapier**, **Make (Integromat)** or **Other**. Click **Create**.

4. **Copy the webhook address**

    Click the integration's row to open it. Next to **Webhook URL**, click **Copy**.

What you choose under **Sent from** becomes the lead source of every lead from this integration, so leads from Zapier show the source "Zapier" in your lists and in [Analytics](/analytics).

## Send leads to it

Paste the **Webhook URL** into your other tool wherever it asks where to send submissions:

- In Zapier, add a "Webhooks by Zapier" action with the POST event, and paste the address as its URL.
- In Make, add an HTTP module that makes a request, set the method to POST, and paste the address as its URL.
- In a WordPress form plugin, such as Elementor Pro's forms, add a webhook action and paste the address. Contact Form 7 has no webhook option of its own, so it needs an add-on that sends submissions to a webhook.

Relivo accepts a POST request with a JSON body or URL-encoded form data, and reads common field names on its own: `email`, `phone`, `first_name`, `name`, `message` and many more. If your tool lets you name the fields, use the names in [Webhook field names](/lead-capture/webhook-field-names).

Then send a test from your tool, and reload the integration's page in Relivo to see it under **Recent submissions**. See [Check recent webhook submissions](/lead-capture/check-webhook-submissions).

Anyone who has the webhook address can send leads into your account. Keep it private, the way you would a password. The **Endpoint key** is the last part of the address; click **Show** to see it.

## Delete an integration

Click the menu button (three dots) on the integration's row, then **Delete**. Its webhook stops working at once. Contacts it already brought in stay. To start again, create a new integration and give your tool its new address.
