Often the submission did arrive, but it landed on someone you already have. Check that first, then work through the rest.

## The person is already a contact

Relivo matches every submission to your contacts by email, then by phone. When it finds a match, it adds the submission to that person's timeline, and updates their phone and address with any the form sent, instead of creating a second contact.

- Search **Contacts** for the person's email or phone, and open their timeline to see the submission.
- No new-lead alert is sent for someone you already have. See [New-lead texts or emails aren't arriving](/troubleshooting/new-lead-alerts-arent-arriving).
- A form submission doesn't turn a contact into a lead, so they don't appear on **Leads**. To follow up with them as a lead, open them, click the more actions button (three dots) and choose **Convert to active lead**.

## A lead form on your website

1. **Check the form is on**

    Open **Capture** > **Lead forms**. A form marked **Off** takes no submissions. Open its menu (three dots) and choose **Turn on**.

2. **Check the count**

    Each form shows how many leads it has taken and when the last one came in. If the count goes up when you test, the form works and the lead is in Relivo.

3. **Check the embed code**

    Click **Embed code** and paste it into the page again, in full. A form deleted in Relivo stops working on your site, even if the code is still there.

4. **Test it yourself**

    Fill in the form on your website with a name and an email you don't use for any contact, then look for that person on **Leads**.

If a visitor sees "Could not verify this submission", the form's spam check didn't pass in their browser. Ask them to reload the page and send it again.

## A form integration (Zapier, Make or a webhook)

1. **Look at Recent submissions**

    Open **Capture** > **Form integrations** and click the integration to open its details. Each submission under **Recent submissions** says **Contact created** or **Couldn't add**. **Contact created** also shows when the submission was added to a contact you already had.

2. **Nothing listed**

    Relivo never received it, or it was an exact copy of a submission received in the last 5 minutes, which Relivo ignores and doesn't list. Copy the **Webhook URL** into your tool again, and make sure the tool sends a POST request with JSON or URL-encoded form fields. 3. **Couldn't add**

    Hover over the words to see the reason, then send a new test. If it fails again, [contact support](/contact) with the integration's name and the reason shown.

A lead named "Unknown", or a line called "Unnamed", means Relivo couldn't find a name in what your tool sent. Match your tool's field names to [Webhook field names](/lead-capture/webhook-field-names).
